Terms of Service

The agreement between you and FillMap: what the service is, what it is not, and the limits on relying on it.

These Terms of Service (“Terms”) govern your access to and use of the FillMap website and service at fillmap.app (the “Service”), operated by FillMap Technologies LLC (“FillMap”, “we”, “us”). By creating an account or using the Service, you agree to these Terms. FillMap accounts are individual: each account belongs to one person, and these Terms are between that person and us. If your employer reimburses you, that is between you and them.

1. The Service

FillMap is a reference tool that helps you look up what a health plan covers. It gathers drug lists and plan details that Medicare, state Medicaid programs, and insurers publish, and answers questions such as: is this medication on the plan's covered list, does the plan require prior authorization, step therapy, or impose a quantity limit, what cost sharing has the plan published for the tier, and what covered alternatives exist. For Medicare plans it also shows which pharmacies a plan lists in its network, which of those the plan treats as preferred, and how that choice changes the published amount. FillMap is a publisher of reference information. It is not an insurer, a pharmacy, a healthcare provider, or a benefits administrator, and it cannot approve, deny, or change your coverage.

2. Not medical advice — and not a price

THE SERVICE PROVIDES REFERENCE INFORMATION ONLY. IT IS NOT MEDICAL ADVICE, IS NOT A SUBSTITUTE FOR PROFESSIONAL MEDICAL JUDGMENT, AND IS NOT A GUARANTEE OF COVERAGE, PAYMENT, OR BENEFITS.

In plain terms: never start, stop, skip, or switch a medication because of something you read on FillMap. Talk to your doctor or pharmacist first. The “covered alternatives” we show are different medications that your plan happens to cover — they are a starting point for a conversation with your doctor, not a recommendation, and they may not be appropriate or safe for you.

What we show about cost is published information and an estimate built from it — never a quote. We show the cost sharing a plan has published for a tier, and, where a plan publishes a negotiated price, what a percentage works out to in dollars, what a list of medications comes to in a month, and what it comes to across a year. Every one of those is arithmetic over published figures. What you are actually charged depends on your deductible so far, any help you receive with drug costs, which pharmacy you use, how many days you are dispensed, and exceptions your plan grants — so the real figure can differ. Covered does not mean free, and a tier is not a price. Drug lists, networks, and plan rules change often, and the data we display may lag or contain errors. Your plan and your pharmacy always have the final say — confirm with them before relying on anything here.

3. We don't want your health information

The Service is designed to work without knowing anything about you. You look up a plan and a medication; we never ask who the medication is for. Please do not enter, upload, or send us identifying or health information — your own or anyone else's — including names, dates of birth, member or policy numbers, medical record numbers, or diagnoses. There is no field for them and no reason to provide them.

Because we do not handle protected health information on behalf of a healthcare provider or plan, FillMap is not a HIPAA covered entity or business associate, and using the Service does not create a business-associate relationship.

4. Accounts

  • You must be 18 or older to create an account. You must provide accurate information and keep your credentials secure.
  • One account, one person. An account is licensed to a single individual human user. Do not share credentials, and do not resell or sublicense access. You are responsible for all activity under your account.
  • The Service is free and requires no payment. There is no plan to choose and no card to enter.
  • An account is optional. You may use the Service as a guest, without registering. A guest session exists only in the browser you created it in, is not recoverable, and saves nothing. Guests have the same daily allowance of lookups as registered accounts, but cannot use the in-app support inbox — we would have no way to reply. Creating repeated guest sessions to get around the allowance is a breach of Section 6.
  • One active session per user. Because access is per user, only one device can be signed in to an account at a time. Signing in on a new device signs the account out of the previous one.
  • API keys. Programmatic access, where FillMap makes it available, is via API keys. A key is personal to the account it is issued for, must be kept secret, is subject to the same lookup scope and rate limits as the app, and may be revoked. Do not share keys or use another person's key.

5. Fees

  • The Service is currently free. There is no subscription, no fee, and no payment method required to create or keep an account.
  • We may introduce paid plans in the future. If we do, we will give you at least 30 days' notice in advance, and you would have to affirmatively choose a paid plan — we will not charge an existing free account automatically, and we will never store a payment method you did not give us.
  • Because nothing is charged, there is nothing to refund and nothing to cancel. You can stop using the Service, or delete your account, at any time.
  • Free does not mean unlimited: the Service applies per-account rate limits and the protections in Section 6.

6. Acceptable use

You agree not to:

  • violate any law or third-party right while using the Service;
  • probe, disrupt, or circumvent security or access controls (including rate limits, single-session enforcement, or the lookup scope);
  • scrape, bulk-export, enumerate, resell, or redistribute the Service or its compiled dataset, or access it by any automated means other than a personal API key used within its limits;
  • use the Service to build a competing product;
  • introduce malware or interfere with other users' use of the Service.

The Service applies per-account rate limits and monitors usage for abuse. We may throttle, and we may automatically lock an account whose usage is consistent with bulk extraction, pausing access pending review. Locked users are shown the reason and can request a review from within the app. Access is restored if the review clears; accounts used to violate this section may be suspended or terminated.

7. Data accuracy, sources, and availability

FillMap does not create coverage data. It aggregates information published by third parties — principally the Centers for Medicare & Medicaid Services (CMS), the Texas Health and Human Services Vendor Drug Program, and individual insurers' federally required machine-readable formulary files. We keep our copy synchronized with those sources (re-checking and re-ingesting them when they change) and each answer shows an “as of” date and its source.

The data is not, and cannot be, 100% accurate or complete. Our checks confirm that our copy matches what a source has published — not that the source itself is correct, current, or exhaustive. In practice this means:

  • Formularies and coverage restrictions change frequently, and a publisher's file may lag reality or contain its own errors or omissions.
  • Sources update on their own, differing schedules (for example, some monthly, some weekly, some at each insurer's discretion), so different segments and plans may be current as of different dates.
  • A given plan, drug, or “alternative” may be missing, delayed, mismatched, or shown differently than the plan would actually adjudicate it — including where a source is temporarily unavailable, incomplete, or does not publish the data at all. The known limitations, and their scale, are itemized in the Data Disclosures.

Cost sharing is often absent, and is never inferred. Issuers must publish a drug tier for every covered drug, but the cost sharing attached to that tier is optional and many omit it. Where an issuer omits it, we use the cost-sharing data CMS publishes for the same plan and tier, and we show the standard amount — the one applying to enrollees who receive no cost-sharing reduction; if you receive one, you will pay less. Where a tier's own published name states that it carries no cost sharing, we report $0 for that tier. Where a plan numbers its tiers and its published cost sharing is recorded against named categories instead, we report no price rather than infer which number corresponds to which category. The Data Disclosures set out the scale of each of these.

Some medications cannot be searched by name. Insurers identify drugs on their formularies by RxNorm code. For a minority of those codes, the drug name is owned by a commercial drug-information vendor that licenses it, and the U.S. National Library of Medicine is not permitted to publish the name alongside the code. Where that happens we hold the coverage record but have no name to attach to it, so the medication will not appear when you search, will not be listed among a plan's covered forms, and will not be offered as an alternative — even though the plan does cover it. As of September 2026 this affects roughly 660 of the codes on marketplace formularies (about 6%), and it affects marketplace plans far more than Medicare or Medicaid. A medication missing from FillMap is therefore not evidence that your plan excludes it; your plan or pharmacy can confirm.

Accordingly, the Service and its data are provided without any warranty of accuracy, completeness, timeliness, or availability. Always confirm coverage, any conditions attached to it, and what you will be charged directly with your plan or pharmacy before relying on it, and talk to your doctor or pharmacist about the medication itself (see Section 2). We may modify, suspend, or discontinue features or data sources with reasonable notice where practicable.

8. Intellectual property

FillMap and its software, design, and compiled datasets are owned by FillMap or its licensors. We grant you a limited, non-exclusive, non-transferable right to use the Service for your own personal, non-commercial reference while your account is active. Coverage information is derived from data published by the Centers for Medicare & Medicaid Services (CMS), the Texas HHSC Vendor Drug Program, individual insurers, the U.S. National Library of Medicine (RxNorm), and NPPES. That underlying data remains subject to its own terms and conditions, and FillMap is an independent product, not affiliated with or endorsed by those agencies.

9. Termination

You may stop using the Service at any time. We may suspend or terminate access for breach of these Terms (including sending us health information or violating acceptable use), or where required by law. Sections 2, 3, and 10–13 survive termination.

10. Disclaimer of warranties

THE SERVICE IS PROVIDED “AS IS” AND “AS AVAILABLE” WITHOUT WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED, INCLUDING MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, ACCURACY, AND NON-INFRINGEMENT.

11. Limitation of liability

TO THE MAXIMUM EXTENT PERMITTED BY LAW, FILLMAP WILL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES, OR FOR ANY LOSS OF PROFITS, DATA, OR GOODWILL. OUR TOTAL LIABILITY FOR ALL CLAIMS RELATING TO THE SERVICE WILL NOT EXCEED THE AMOUNTS YOU PAID US IN THE TWELVE (12) MONTHS BEFORE THE CLAIM AROSE.

12. Indemnification

You will indemnify and hold FillMap harmless from claims arising out of your use of the Service in violation of these Terms, including any health information you send us and any medical decision made in reliance on the Service.

13. Governing law and disputes

These Terms are governed by the laws of the State of Texas, without regard to conflict-of-law rules. Disputes will be resolved in the state or federal courts located in Texas, and you consent to their jurisdiction.

14. Changes to these Terms

We may update these Terms from time to time. Material changes will be announced by email or in-app notice at least 14 days before they take effect; continued use after that constitutes acceptance.
